How to withdraw​​

Taking part in the C4AR Database is entirely voluntary. You are free to withdraw at any time without giving a reason, and your decision will not affect the care you receive.

If you have any questions or concerns before making your decision, please get in touch - we're always happy to help. If you decide to withdraw, simply email us at C4ARD@ukkidney.org

To help us identify your record, please include your NHS number (or enough information for us to identify you). Once we have received your request, we will confirm that your withdrawal has been processed and you will not be contacted again.

Unless you ask us to delete it, we would like to continue using the information already collected, as it remains valuable for research. If you would prefer us to remove your data completely, just let us know.
